MARION COUNTY — The Marion County Health Department will offer school and sport physicals by appointment on Friday July 31st at the Salem office, located at 118 Cross Creek Blvd., and on Friday August 7th at the Centralia office located at 1013 N. Poplar. The fee is $40.00.

Payment may be made with cash, check, credit card or debit card. Medicaid, Blue Cross Blue Shield, Aetna, Coventry, Humana, Meridian, Health Alliance, Healthlink, Cigna and Land of Lincoln insurances will be accepted.
Immunizations will be offered for an additional fee.

For school year 2015-2016: 3 doses of hepatitis B vaccine will be required for children entering 6 through the 12th grade. 2 doses of MMR vaccine will be required for children entering K-12th grade. 1 dose of Tdap vaccine will be required for children entering 6th through 12th grade. 2 doses of Varicella (Chickenpox) vaccine will be required for children entering Kindergarten, 1, 6, 7, 9, and 10th grade. 1 dose of meningococcal vaccine will be required for children entering 6th grade and 2 doses of meningococcal vaccine will be required for children entering 12th grade.
