SALEM — Salem police arrested an area pediatric provider Tuesday night on charges accusing her of retail theft and possession of a controlled substance.

According to Salem police, Tamara Foltz, a Family Nurse Practitioner, turned herself in Tuesday night and was booked into the Marion County Jail.

Police report the arrest includes eight separate incidents from May 26 of this year and July 10 at the Walmart store in Salem. Surveillance video at the store reportedly showed Foltz changing bar codes on some items and simply not scanning other items when going through the self-checkout of the store.

The alleged incidents were reported to the Salem Police Department over the weekend but an investigation had not yet ensued Monday when Walmart employees stopped Foltz for allegedly committing the same actions that day.

Salem police were called and Foltz was released after being issued a notice to appear on misdemeanor retail theft.

However, on Tuesday a review of surveillance videos dating back to May 26 allegedly revealed the other incidents and brought the alleged total of items taken to more than $500.

Foltz was contacted about the allegations and turned herself in Tuesday night. Police say that as she was being booked into the Marion County Jail a pill bottle was found in her possession with an assortment of prescription pills without a prescription.

Police say the State’s Attorney’s Office has indicated they won’t charge the controlled substance offense until and unless a lab report confirms what pills were seized in the arrest.

UPDATE: Foltz was formally charged Wednesday in Marion County Court with one Class 3 felony count of retail theft.
